At the time of writing this blog post, the USPS Form 1583 (Application for Delivery of Mail Through Agent) was updated June 2024. I can provide June 2024 1583 Help! The form has been updated, and I've observed that many websites contain outdated information about it. This blog post will clarify the fields in the form. Always refer to the USPS website for the most current version. Be sure to follow the official instructions provided on the form and the USPS website and from your mailbox provider. Everything in this post applies to virtual mailbox providers as well as traditional brick and mortar providers.
In providing the information below, I am assuming YOU are the applicant. Please keep this in mind in case you are not the applicant.

Item 1 (Private Mailbox Information) - This section will state the date your Private Mailbox (PMB) was opened. It also has a field when the PMB was closed. This field will need to be updated if you are seeking to close your PMB. You will need to contact the mailbox provider for more information.
Item 2 (Commercial Mail Receiving Agency (CMRA) Place of Business Information) - This section will detail your mailbox provider's address for delivery of mail. Make sure to include the house number, street, and apartment/suite number for the mailbox provider, if applicable. Item 2b will list your PMB number.
Item 3 (Type of Service Requested) - This section will specify whether you are using your PMB for business use or personal use. If you select "Business/Organization Use," you will need to complete item 7. If you select "Residential/Personal Use," you will need to complete a separate PS Form 1583 (Application for Delivery of Mail Through Agent) for each adult using this PMB.
Item 4 (Name of Applicant) - This section will detail the information of the person who is filling out the application. Please be sure to include the applicable house number, street, and apartment/suite number for the applicant's home address. Further, this address must match the verification document provided in item 9b.
Item 5 (Authorized Individual) - Should you want to authorize someone other than you to be able to collect mail from your PMB, you will need to complete this item. Please be sure to include the applicable house number, street, and apartment/suite number for the authorized individual's home address. Further, this address must match the verification document provided in item 11b.
Item 6 (If Transferring PMB Mail to Another Address) - Complete item 6 if the mail addressed to this PMB is to be transferred, mailed, shipped, or emailed to another address. Please be sure to include the applicable house number, street, and apartment/suite number for the address of which the mail will be transferred, mailed, or shipped.
Item 7 (Business/Organization Information) - If "Business/Organization Use" was selected in Item 3, then you will need to fill out the information required in item 7. Please be sure to include the applicable house number, street, and apartment/suite number for your business's/organization's address. Item 7i (Place of Registration) means the country and state (if domestic), or the country (if foreign) of the company/organization.
Item 8 (Photo ID Information for Applicant) - This item requires you to provide a copy of your photo ID. Please note that two types of identification are required for the applicant. One ID must be a government-issued photo ID. The second must confirm the applicant's address listed in item 4f. The acceptable types of photo ID are listed in item 8e. Please make sure to attach a copy of the photo ID document. Please note that although the driver's/nondriver's ID is listed in 8e and 9g as an option for both the applicant's photo ID and address ID, it may be used for only one of the IDs (either photo ID or address ID), not for both.
Item 9 (Address ID Information for Applicant) - This item requires you to provide ID information to confirm your address listed in item 4f. The acceptable types of address verification documents are listed in item 9g. As noted in item 8 above, you cannot use your driver/nondriver ID in both item 8e and item 9g. Please make sure to attach a copy of the address ID document.
Item 10 (Photo ID Information for Authorized Individual (if applicable)) - Similar to Item 8, if you are listing an authorized individual in item 5, this item requires you to provide a copy of a photo ID of the authorized individual. Two types of identification are required for the authorized individual. One ID must be a government-issued photo ID. The second must confirm the authorized individual's address listed in item 5f. The acceptable types of photo ID are listed in 10e. You will need to attach a copy of the photo and address ID documents. Please note that although the driver's/nondriver's ID is listed in 10e and 11g as an option for both the authorized individual's photo ID and address ID, it may be used for only one of the IDs (either photo ID or address ID), not for both.
Item 11 (Address ID Information for Authorized Individual (if applicable) - This item requires you to provide ID information to confirm the address of the authorized individual listed in item 5f. The acceptable types of address verification documents are listed in 11g. As noted in item 10 above, you cannot use your driver/nondriver ID in both item 10e and 11g. Please make sure to attach a copy of the address ID document.
Item 12 (Exceptions for Additional Recipients of Mail) - This item is optional and only needs to be filled out as applicable. For those who selected "Business/Organization Use" in item 3, please list the members who will be receiving mail at this PMB. Each person listed must, upon request, present two forms of valid ID to the Postal Service. For those who selected "Residential/Personal Use," a parent or guardian may receive the mail of a minor by listing the minor's name -- the minor's ID is not required.
Item 13 (Signature of Applicant) - Your signature and date you signed! By signing the form you certify the following: "I certify that all information furnished on this form is accurate, truthful, and complete. I understand anyone who furnishes false or misleading information on this form or omits information requested on this form may be subject to criminal and/or civil penalties, including fines and imprisonment." If you selected "Business/Organization Use" in item 3, an officer must sign the application and provide his or her title.
Item 14 (Signature of CMRA or Authorized Employee) - This item is optional and only needs to be filled out by an agent or an authorized employee of the mailbox provider if you are signing in the mailbox provider's presence. You can alternatively sign in front of a notary, like myself!
As required by Form 1583 (Application for Delivery of Mail Through Agent), you must sign or confirm signature in the physical or virtual presence (in real-time audio and video) of the mailbox provider or the mailbox provider's employee OR acknowledge your signature in the physical or virtual presence (in real-time audio and video) of a notary public.
